Note: we are still in the process of updating our forms and documents with the new name and logo. So, you will find our forms and documents still use the name Spirit Equestrian.

General Forms & Files 

Download and print the forms you need at Fox Canyon Farm, then drop them in the locked mailbox by the office or near the hub by the shop (maintenance form). Changes are processed M-F. If a form is fillable, just fill out online, print, sign (if needed) and scan & email to us at accounting@spiritequestrian.com.

• Request for Service Form
  Use this form for requesting any horse & office related services. Feed changes, adding services like blanketing or turnouts, updating information, etc. Please give 72 hour notice. If you need service sooner, please fill out the form AND make direct contact with us to see if we can accommodate your request.

• Maintenance Request & General Communication Form - Fillable
  Use this form to alert us to a variety of issues. If there is a true emergency, don't hesitate to alert us via phone. For all other requests, fill out this form. Non-emergency repairs M-F only.

New Client Forms

Download and print your required client forms. If a form is fillable, just fill out online, print, sign (if needed), scan, and email to us at allyson@foxcanyonfarm.com 

• New Horse Checklist – Fillable
  Use this form as an overview to be sure you haven't forgotten anything. The office will sign off as documents and payments are received.

• Boarding Agreement – Fillable
  Standard boarding agreement for a stall at Fox Canyon Farm (formerly Spirit Equestrian).

• Liability Waiver – Fillable
  We require this form signed by anyone who will have contact with your horse. Keep a copy of this form in your tack trunk for guest riders and friends who will help you.

• Horse ID & Feed Form - Fillable
  Let us know what needs your horse may have before its arrival.

• Emergency Contact Form – Fillable
  Useful in case of emergency for you and your horse. Let us know who to call.

• Trailer Parking Form
  Use this form if you will be parking a trailer in our trailer lot.

• Credit Card Authorization Form (Boarders) - Fillable
  Use this form if you would like to pay via credit card (one time or recurring)

Please note: a $400 security deposit will be collected prior to arrival at Fox Canyon Farm. We will return your security deposit minus any outstanding balances via mail after your horse's departure.

We require proof of minimum vaccinations before arrival at Fox Canyon Farm.
